Entry Fee:  $525 for U09-U10     $550 for U11-U12     $575 for U13-U15       $595 for U16-U19

 
 
Deadline:  Sunday, February 13  
 
Application fees must be submitted with package if paying by check.  Mailed applications without application fee will not be accepted. 
 
Faxed and Emailed Applications can not be accepted. Application Form MUST be submitted online.
